Covid-19 Mediation by video conference: consent

The parties have agreed to mediate Canada Industrial Relations Board file number XXXXX-C by video conference.

As a participant in the mediation, I undertake, acknowledge, and consent to the following:

  1. The mediation will take place using the video conference platform Zoom;
  2. Only the persons who have been identified to the mediator as attending, shall log into the Zoom call;
  3. The participants will not record the mediation session;
  4. The password provided for the video conference will not be shared with anyone who has not been invited to participate by the mediator without the consent of the mediator;
  5. Although steps will be taken to protect the confidentiality of the mediation by using security features in Zoom, the nature of the technology being used is such that confidentiality cannot be guaranteed; 
  6. Participants must have access to a telephone during the mediation and be ready to proceed by teleconference if there is a technical or other problem;
  7. To minimize the risk of security breaches, participants will not open attachments or click on links in Zoom that they are not certain are valid and for the purposes of the mediation;
  8. Sensitive documents should be exchanged over Zoom;
  9. By signing below or providing confirmation by email, the participant affirms their consent to the terms and conditions laid out in this document.
